Initial meeting between Client, Family, and Partner. Introductions and getting to know one another. What can be expected from your Partner and an overview of the process.

Biannual Reassessment

Twice-yearly (or as needed) review of Client's Plan of Care. Updates, modifications, and general review will be acknowledged and updated. Plan of Care will be created

Care Management Visits

Concierge-style visit personalized to each Client's individual medical and psychosocial needs. Plan Of Care Development meetings fall in this category.

Essential Medical Document Review

Living Will, Advanced Directives, POLSTs.
